You Have a Choice Where You Live!

  • If you have lived in a nursing facility in Cook County for at least 60 days
  • And you are 18 years or older
  • And you qualify for Medicaid

You are a Class Member and part of the Colbert Consent Decree!

The Colbert Consent Decree is the result of a lawsuit against the State of Illinois, because Mr. Colbert wanted more options for care other than nursing facilities. The result of the lawsuit is that the State of Illinois agrees to provide support and services to help Class Members (like you) move to the community, with the necessary support and assistance you need.

How H.O.P.E. Can Help You

H.O.P.E. can help you with many things:

  • Build the skills you need to care for yourself in the community.
  • Identify and arrange for services and supports to help you.
  • Apply for income support and/or help you find a job.
  • Find affordable housing in a neighborhood you would like to live in.
  • Get housing, financial assistance, furniture, and make sure your new living space meets your needs.
  • Make changes to your housing unit and get the items you need to support your physical needs.
  • Connect you to programs and services to help you manage your mental health and substance use safely
  • Set up doctors and specialists in the community with you.
  • Work with you for a year and a half after you move to ensure your safety and success.

What Happens Next

If you’re interested:

A H.O.P.E. Team Member will perform an outreach by asking to meet with you. They’ll tell you more about H.O.P.E. and answer your questions.

If you agree, a H.O.P.E. Assessment Team Member will ask you a lot of questions about yourself and your history. This information allows us to help you put together a plan to safely move back to the community.

After they know more about you, they’ll help you work on your goals and your plan to move closer to your ultimate goal — Moving Back to the Community.

H.O.P.E. is here for you. We work closely with your nursing facility care team to ensure that your rights are protected. For more information on nursing home residents’ rights or to file a complaint:  Long-Term Care Ombudsman Program (illinois.gov)

There is no cost to you to participate in our program.

  • Ask your Social Service Director.
  • Talk to a H.O.P.E. Team member when they’re in your facility.
